SERVICES
1. Application for Marriage License
Who may avail: Couples who want to marry; one must be a resident of Concepcion.
Requirements:
Birth certificates (certified true copy/photo copy)
CENOMAR (Certificate of No Marriage)
Pre-marriage counseling certificate
Parent’s advice (21–24 yrs. old) / Parent’s consent (18–20 yrs. old)
Proof of civil status (death certificate, annulment papers, divorce decree, if applicable)
For foreigners: Legal capacity to contract marriage / affidavit, passport (with arrival page)
Steps: Submit information sheet & requirements → Verification & pay ₱350 → Attend counseling/seminar → Submit certificate & pay ₱200 → Wait 10-day posting → Release license
Fee: ₱550 (₱350 + ₱200)
Processing Time: 20 minutes (+ 10-day posting)
2. Issuance of Certified True Copy of Civil Registry Documents (Birth, Marriage, Death, etc.)
Who may avail: Concerned person or authorized representative.
Requirements:

Valid ID of concerned party

Authorization letter (if representative)
Steps: Request verification → Pay required fee → Present O.R. & receive certified copy
Fee: ₱100 (local use) / ₱150 (travel abroad) / ₱60 (certified photocopy)
Processing Time: 14 minutes
3. Registration of Birth, Marriage, and Death
Who may avail:
Birth: Child born in Concepcion

Marriage: Contracting parties married in Concepcion

Death: Deceased whose death occurred in Concepcion
Requirements:
4 copies of accomplished Certificate (Birth/Marriage/Death), properly signed with no erasures

For illegitimate but acknowledged child: Affidavit of Acknowledgment/Admission of Paternity

For Death: Municipal Health Officer’s certification of cause of death
Steps: Submit documents → Verification → Issuance of registered document

Processing Time: 5 minutes
4. Delayed Registration of Birth, Marriage, and Death
Who may avail: Persons whose birth, marriage, or death was not registered within 30 days in Concepcion.
Requirements:

Birth: Affidavit of delayed registration, supporting documents (baptismal, school, medical, voter’s ID, affidavits of parents/disinterested persons, PSA certificate of no record)

Marriage: Affidavit of delayed registration, certified marriage license application, PSA certificate of no record

Death: Affidavit of delayed registration, burial/cremation certificate, PSA certificate of no record
Steps: Submit documents & info sheet → Verification & posting for 10 days → Pay required fee → Receive approved document
