  • Accomplished RSBSA Form
  • Barangay Certification or Land Title
  • 2×2 Picture
  • Valid ID (Photocopy)

Steps:

  1. Sign in the Client Log Book
  2. Request RSBSA form → Interview and preparation
  3. Accomplish form → Review requirements
  4. Submit application → Encoding and submission to provincial office

  • Certificate of Good Standing
  • FA/FO/IA/RBO Profiles
  • Business Plan
  • MAO/CAO/PAO Endorsement

For DA Accreditation:

  • Association Profile
  • Omnibus Sworn Certification
  • Endorsement Letter
  • Letter of Intent
  • Board Resolution
  • Transmittal for Accreditation

Steps:

  1. Sign in the Client Log Book
  2. Submit documents → Review completeness
  3. Preparation of endorsement letter
  4. Release of endorsement letter
