Who may avail: Mothers with newborns & infants
Requirements: Vaccines offered – BCG, Penta (1,2,3), OPV (1,2,3), Measles, MMR, IPV, PCV (1,2,3), Hepatitis B, Tetanus Diphtheria (TD)
Steps: Secure registration number & growth chart check → Immunize infant & advise next check-up

3. Dental Services

Who may avail: Walk-in clients

Requirements: None

Steps: Registration & vital signs → Dental treatment (simple extraction: 10 minutes / complicated: 45 minutes) → Provide medicine or prescription if necessary

4. Issuance of Sanitary Permit and Health Certificate

Who may avail: Any client applying for a sanitary permit & health certificate

Requirements (depending on establishment type):
Non-food establishments (store, hardware, feed supply, etc.): Chest x-ray, urine, stool, Mayor’s permit application

Food establishments (bakeries, canteens, groceries, etc.): Same as above
Food processing: Same as above

Poultry/piggery/cemetery: Same + Barangay Resolution

Establishments with GROs (videoke bars): Chest x-ray, urine, stool, Hepa B result, Pap smear, Mayor’s permit, Barangay clearance, 1×1 picture, occupational tax
Water refilling stations: Chest x-ray, urine, stool, Mayor’s permit, water site clearance, potability certificate (monthly/bi-annual tests), isometric plan, operational permit

Public places (schools, hospitals, hotels, etc.): Chest x-ray, urine, stool, Mayor’s permit application

Steps: Apply with requirements & inspection → Pay ₱50 at MTO → Present OR & claim permit/certificate
