2. Issuance of Certificate of Indigency
Who may avail:
Indigent families of Concepcion, Tarlac
Requirements:
- Certificate of Indigency (Original from Barangay)
Steps:
- Present requirement → Interview and verification → Preparation of certificate
- Review and signing → Release of certificate

3. Issuance of PWD ID
Who may avail:
Residents of Concepcion, Tarlac with permanent disability
Requirements:
- Medical Certificate with recommendation for PWD (Original)
- Certificate of Disability (Original)
- 2×2 picture with white background
- Valid ID (Photocopy) / Birth Certificate if minor
- If claimant is immediate family: Valid ID + Birth/Marriage Certificate
- If claimant is not immediate family: Valid ID + Certificate of Guardianship

- Medical Certificate of patient (Photocopy)
- Barangay Certificate of Indigency (Original)
- Valid Government ID (Photocopy)
Steps:
- Submit requirements → Assessment → Issuance of intake sheet
- Submit accomplished intake sheet → Interview → Review and signing
- Receive Certificate of Financial Capability

5. Issuance of Solo Parent ID (RA 11861)
Who may avail:
Residents of Concepcion, Tarlac who are qualified solo parents
Requirements:
Requirements vary depending on category (e.g., death of spouse, separation, abandonment, OFW, unmarried parent, guardian, pregnant woman, etc.), but generally include:
- Birth Certificate/s of child/children
- Marriage Certificate (if applicable)
- Death Certificate / Legal documents (if applicable)
- Affidavits (parental care, residency, etc.)
- Proof of income / ITR or Certificate of Unemployment
- Barangay Certification
